Your First Visit
What to Expect When You Call the Clinic:
When you call the clinic, you will be asked to provide some basic information about yourself, after which you will be able to begin our intake process. The intake process involves a 20-30 minute phone screening to help us evaluate your needs, followed by a two-hour in-person intake with a Gronowski student therapist. The intake process is typically finished within two weeks of your first contact with Gronowski.
What to Expect in Your Intake:
During your initial intake, your therapist will gather information about you, your problems and your history and will be able to answer your questions regarding services, fees, confidentiality and what to expect in therapy. You will also be asked to review and sign a form ("Consent to Psychological Treatment") describing important office policies such as handling of emergencies, protection and use of confidential information and limits to confidentiality as well as a "Notice of Privacy Policies" which tells you about how your information can be used.
We are interested in developing a collaborative relationship with you in which we try to decide together what kind of treatment would be best for you. However, not everyone will be referred for continuing therapy at the clinic. We make decisions on what we believe will be best for you.
You can save time in your first appointment by completing the forms below and bringing them to your visit.
- Consent to Psychological Treatment
- HIPAA Notice of Privacy Policies
- Client Questionnaire
- Demographic Sheet
We look forward to seeing you.
