Degree/Enrollment Verification
Students and alumni may request certification of their degrees, dates of attendance, full-time enrollment, and licensing forms by completing the Degree/Enrollment Verification Form. Processing time can be up to five business days.
Please note: All requests must contain the following information. Missing information could delay a request.
- Full legal name
- Any former names
- Date of birth
- Phone number
- Complete name and address of each recipient
- Specify information that needs to be verified (i.e. degree verification, enrollment verification, dates of attendance, etc.)
- Legal signature to authorize the release of enrollment/degree verification
Third Party Requests:
All degree verifications must be accompanied by the student’s signed “Consent Release of Information Form.” Verification requests may be faxed, emailed, or mailed to the following address:
