What Evidence Favors Actuarial Assessments in Comparing Violence Risk Assessment Tools?
The actuarial approach to violence risk assessment emerged as a corrective to unstructured clinical judgment, which research had consistently shown to be subjective, inconsistent, and sometimes no more accurate than chance. Actuarial instruments such as the Violence Risk Appraisal Guide (VRAG), the Static-99, and the Sex Offender Risk Appraisal Guide (SORAG) assign numerical values to predetermined risk factors and combine them algorithmically to produce a total score. That score is then referenced against normative tables to generate a probabilistic estimate of recidivism over a specified period. The appeal is straightforward: human judgment biases are removed from the decision-making process, and each individual is appraised using the same criteria regardless of who conducts the assessment.
Early meta-analyses supported the efficacy of actuarial violence risk assessment tools. A widely cited meta-analysis of 136 studies found that statistical predictions were approximately 10 percent more accurate than clinical judgments, with dramatically better accuracy in roughly one-third of comparisons (Grove et al., 2000). A subsequent meta-analysis of 67 studies concluded that actuarial assessment was 13 percent more accurate than clinical judgment overall and 17 percent more accurate for predictions of violent or criminal behavior (Ægisdóttir et al., 2006). A score that predicts a 40 percent probability of violent recidivism in a development sample does not necessarily mean that any particular person with that score has a 40 percent chance of reoffending, particularly if local base rates, treatment availability, or supervision conditions differ from the original study population.
What Should Forensic Psychologists Know About the SPJ Model and How It Compares to Actuarial Violence Risk Assessment Tools?
The structured professional judgment approach was developed in the 1990s to address these limitations while preserving the benefits of empirical grounding. SPJ tools, including the HCR-20 V3, the SARA-V3, the SAVRY, and the START, guide evaluators to consider risk factors with demonstrated empirical support, rate their presence and relevance, and then use professional judgment to arrive at a summary risk rating. Unlike actuarial instruments, SPJ tools do not impose strict cutoffs or algorithms to determine risk level. Instead, they provide a framework within which the forensic psychologist exercises clinical discretion anchored in research.
The SPJ approach incorporates several features that actuarial tools generally lack. SPJ requires evaluators to consider the relevance of each risk factor to the individual case, not merely its presence. It includes dynamic risk factors that change over time and can be targeted in treatment. It directs evaluators toward risk formulation, the process of developing a narrative theory of why this person might be violent under what circumstances, rather than generating a single number. And it structures the development of risk management recommendations that directly inform institutional decisions, treatment planning, and supervision conditions.
Guy's (2008) meta-analytic survey of SPJ performance, encompassing 113 studies, provided the first comprehensive evidence that the SPJ model produces predictive validity comparable to actuarial instruments, particularly when evaluators' summary risk ratings (rather than mechanically summed item totals) are used as the predictor. This finding was significant because it demonstrated that the clinical judgment component of SPJ, when structured by empirically validated guidelines, does not degrade predictive accuracy.
What Do Major Meta-Analyses Comparing Violence Risk Assessment Tools Show?
The most rigorous comparative evidence comes from several large-scale meta-analyses published between 2011 and 2025. Their findings suggest that fitting assessment tools to the population is at least as important as which tools the forensic psychologist uses.
Singh, Grann, and Fazel (2011) conducted a systematic review and metaregression of 68 studies involving 25,980 participants, examining nine commonly used risk assessment instruments across both the actuarial and SPJ categories. The study used multiple performance statistics rather than relying solely on the Area Under the Curve (AUC), which the authors argued was insensitive to meaningful differences in tool performance. Their findings indicated that actuarial tools and SPJ instruments performed comparably in predicting violence. The tools designed for specific populations and outcomes performed better than general-purpose instruments, and predictive validity was higher in older samples and when the outcome was violent offending rather than general criminal behavior.
Fazel, Singh, Doll, and Grann (2012) published a follow-up analysis, stratifying instrument performance by whether the tool was designed for violent, sexual, or general criminal offending. Drawing on 73 samples involving 24,827 people, this meta-analysis confirmed that structured tools of both types predicted violence above chance levels but also highlighted the limitations of all current instruments. The mean positive predictive value for violent offending tools was .41, meaning that among individuals classified as high risk, fewer than half went on to commit violent offenses. The negative predictive value was considerably stronger at .91, indicating that low-risk classifications were accurate in the vast majority of cases. The type of instrument, whether actuarial or SPJ, was not a significant moderator of predictive accuracy.
More recently, Viljoen and colleagues (2025) conducted a pre-registered meta-analysis directly comparing the predictive validity of risk assessment tools to unstructured judgments, using a three-level meta-analytic approach. Their findings further supported that structured tools outperform unstructured judgment, and that both actuarial and SPJ approaches contribute meaningfully to prediction quality. Their methodology addressed several limitations of prior meta-analyses, including the tendency to pool effect sizes from different instruments without accounting for within-study dependencies.
Taken together, these meta-analyses undermine the claim that actuarial instruments are categorically superior to SPJ tools for violence risk assessment. The evidence supports what researchers have called the "Dodo bird verdict" in risk assessment: when properly implemented, the major structured approaches perform comparably. The differences that do emerge tend to reflect instrument-specific factors (such as whether the tool matches the population and outcome in question) rather than fundamental advantages of one methodological philosophy over the other.
How Can Forensic Psychologists Synthesize Comparisons and Analyses of Violence Risk Assessment Tools?
If predictive accuracy is roughly equivalent, the choice between actuarial and SPJ approaches should turn on what else the assessment needs to accomplish. For forensic psychologists, this is where the practical implications of the meta-analytic evidence become most significant.
Violence risk assessment in forensic practice is rarely conducted solely for the purpose of prediction. Courts, forensic hospitals, and correctional agencies want to know whether someone is likely to be violent, under what circumstances, through what mechanisms, and what can be done about it. Actuarial instruments are not designed to answer these questions. They generate a number, and that number refers back to a normative group. SPJ tools, by contrast, structure the process of risk formulation, the development of individualized theories of violence risk that connect identified risk factors to scenarios of concern and management strategies. The HCR-20 V3 guides professionals through the conceptualization of violence with an emphasis on intervention and risk management, while the START integrates both strengths and vulnerabilities into evaluations designed to change over time.
This distinction matters in the courtroom as well. Forensic psychologists who testify about violence risk should expect cross-examination about why they chose a particular methodology. An evaluator who used an SPJ approach can point to the meta-analytic evidence showing comparable predictive validity while explaining that SPJ additionally informed case-specific risk formulation and management recommendations. An evaluator who relied solely on an actuarial score may be challenged on the instrument's inability to account for dynamic factors, treatment effects, or contextual changes since the normative data were collected.
The authorship bias findings reported by Singh, Grann, and Fazel (2013) add another layer of complexity. Their meta-analysis found that studies authored by tool designers reported predictive validity findings approximately twice as high as those of independent investigations. None of the 25 studies in which a tool designer was an author disclosed a conflict of interest, despite journal policies requiring such disclosure. This finding applies across both actuarial and SPJ tools and underscores the importance of evaluating instruments based on independent replication data rather than development-sample results. Forensic psychologists should critically evaluate the evidence base for any instrument they use, considering whether the published validation data reflect independent replications in populations similar to the individuals they evaluate.
The cultural applicability of both approaches warrants attention as well. SPJ tools, because they allow evaluators to incorporate case-specific contextual factors, have theoretical advantages when assessing individuals from populations underrepresented in actuarial normative samples. However, the clinical discretion that SPJ provides can also introduce bias if evaluators are not trained to recognize how cultural factors influence risk presentation and behavioral norms. Actuarial instruments face the opposite problem: their rigidity may prevent the identification of culturally specific risk or protective factors, but they are also less susceptible to the evaluator's own cultural biases in scoring. Neither approach resolves the fundamental challenge of cross-cultural risk assessment, but the forensic psychologist should be prepared to articulate how cultural factors were considered in the evaluation.
Conclusion
The meta-analytic evidence accumulated over the past fifteen years offers forensic psychologists a clear answer to a question that once seemed intractable: actuarial and SPJ approaches to violence risk assessment produce comparable predictive validity. The choice between them should therefore be driven by the purpose of the assessment, the referral question, and the population being evaluated rather than by claims of methodological superiority. For forensic psychologists working in settings where risk management recommendations, treatment planning, and individualized formulation are expected, SPJ tools offer advantages that extend well beyond prediction. For contexts where a standardized probability estimate is the primary deliverable, actuarial instruments remain appropriate. In many forensic evaluations, using both approaches in a complementary fashion provides convergent evidence and demonstrates methodological thoroughness. What the evidence does not support is the uncritical adoption of either approach based on outdated assumptions about which is "better." The forensic psychologist's role is to select and apply tools with awareness of their strengths, limitations, and the specific demands of the case at hand.
